Company Policies
The Moorpark Health Care Center welcomes all persons in need of services without regard to age, disability, race, color, creed, national origin, ancestry, religion or sex and will make no determinations regarding admissions, discharges or care based on those factors.
A Patient may have visits from immediate family other relatives and visits from others who visit with his/her consent, during established visiting hours and are subject to the Patient’s right to deny such access or withdraw consent at any time. For the safety of the Patient and others, if any of the Patient’s guests fail to abide by the Facility rules for Visitors, the Patient and/or Responsible Party agree upon the Facility’s request to arrange for prompt removal of such Visitors from the Facility. Daily Visiting Hours are from 11:00 AM to 8:00 PM. Patients who are in critical condition (imminent death) may have family visits outside of regular visiting hours. Visitors must check in with the Nursing Staff and front desk during such situations. Clergy may visit at any time, but should check with Nursing Staff before entering Patient’s room. Visitor parking is in the back of the Facility. All Visitors have to sign the Visitor’s Log before meeting the Patient. Family and friends are welcome to bring food in to eat with their loved ones at meal time. If you do bring food in, please check in with the Charge Nurse. Please make sure to clean up when finished and throw away all trash. Children 12 and under should be accompanied by an adult at all times and should not be left unsupervised. Facility Staff will not be responsible for watching your children during your visit.
Moorpark Health Care Center is a NON-SMOKING Facility. Patients, Employees and Visitors are not allowed to smoke in any area inside the Moorpark Health Care Center under any circumstances. Flammable materials such as matches, lighter fluid, cigarettes or tobacco are not to be given to Patient residents without the consent or knowledge of the Charge Nurse. Please ask the Charge Nurse for designated smoking areas. Thank you for your cooperation.
Employees who accept any type of gratuity will be disciplined for misconduct. Any gifts offered in appreciation should be approved by Administration prior to Family/Patient making any donations. Please contact the Executive Director or Director Of Nursing at Moorpark Health Care Center if you are interested in donating items. Employees are not to accept any donated items without prior authorization by Administration.
Moorpark Health Care Center will not knowingly hire or retain any Employee who has been convicted of a crime or theft within a period of 5 years preceding his/her date of hire. An Employee who is guilty of theft will be dismissed. Employees are not allowed to accept gifts or borrow items from a Patient/family. All Employees are oriented to the theft and loss policy upon hire by the Director Of Nursing or Executive Director. All Employees hired have their certificates and licenses verified by our office by contacting the respective licensing or certifying boards.
No medications or drugs may be brought upon the Facility premises unless the medication or drug is properly labeled. Nothing shall prevent the Facility from requiring the Patient’s pharmacy or other source from complying with the Facility’s policies and procedures reasonably necessary for the care of the Patient or required to meet state or federal regulations.
Due to Safety and Fire Regulations Patients cannot bring appliances of any kind into the Facility without prior written permission of the Facility. The Patient and/or responsible Party releases the Facility from any responsibility for and agree to indemnify and hold the Facility harmless against any claims made by the Patient or by other persons with respect to burns, injuries or property damage which may occur as a result of the Patient’s use of appliances.
The Facility shall make reasonable efforts to safeguard the Patient’s property and valuables that are in the possession of the Patient. The Facility will not be liable for either damages or loss of personal property of the Patient except with respect to damage or loss as a result of negligent acts of the Facility personnel or as required by law.
In order to preserve the value of the Facility’s property for future Patients’ use, the Patient agrees to use care to avoid damaging the Facility’s property and premises. No pictures, posters or other ornaments may be hung on the walls, nor may furniture be rearranged in the Patient’s room without prior consent of the Administrator of the Facility, whose consent shall not be unreasonably withheld. The Patient will be responsible for repair or replacement of the Facility property damaged or destroyed by the Patient. However, the Patient will not be responsible for such damage as is to be expected as a result of ordinary wear and tear.
The Patient’s account shall be billed monthly to the Patient and/or Responsible Party. Bills are due on or before the first day of each month. The Patient and/or Responsible Party agree to pay the assigned daily room and board rate by the first day of each month and agree to pay a late fee, if payment is not received by the 3rd day of the month, in the amount of 10% of the assigned monthly rate. Any remaining pre-paid monthly rent will be reimbursed no later than 2 weeks after discharge or death of the Patient.
